Please note that to attend a Scuba Trust Try-Dive session the Medical Form MUST be completed and returned prior to attendance. If there are any “YES” answers on the Try-Dive Medical Form a member of the training team will be in touch to discuss the form with you.

ÂPADI Course Forms
The PADI Medical Form is required to participate on any PADI course. If any “YES” answers are given a physician MUST fill in then sign & stamp the appropriate sections. This can be done by your own GP or by an approved diving doctor.
BSAC Course Forms
The self declaration form is needed if there are no ‘YES‘ answers. The medical referee form is required for a full medical if any “YES” answers are given. This has to be carried out by an approved diving doctor on the ukdmc list. This can be found HERE
